Al-Qadsiah's origins trace back to 1935, when football teams formed among workers at the Arabian American Oil Company (Aramco) in the Eastern Province. The formal club was established in 1967 through the merger of these local teams in Khobar. The Eastern Province's petroleum economy provided a unique context — a cosmopolitan workforce, international exposure, and the financial resources that come from living at the center of the world's oil industry. By the time the Saudi Professional Football League was established in 1976, Al-Qadsiah were a well-organized club with strong community roots and the ambition to compete at the national level.

Al-Qadsiah were among the inaugural participants in the Saudi Professional Football League in 1976-77, and they maintained an uninterrupted presence in the top flight for 21 seasons — a remarkable run that ended with relegation in 1996-97. Since then, the club have oscillated between the first and second divisions, experiencing the highs of promotion and the lows of relegation multiple times. Despite the inconsistency, Al-Qadsiah maintained a loyal following in Khobar and the broader Eastern Province, and the club's history of top-flight participation gave them credibility even during spells in the lower divisions.

In 2023, Saudi Aramco acquired Al-Qadsiah, transforming the club's prospects overnight. The world's largest oil company brought not just financial resources but also the organizational expertise and global ambition that characterize Aramco's other ventures. Robbie Fowler was initially hired as manager, followed by Spanish international Míchel, who led the club to promotion to the Saudi Pro League in 2024. For the 2025-26 season, former Liverpool and Leicester City manager Brendan Rodgers was brought in to lead the club's challenge at the highest level. Investment in the squad, coaching staff, and infrastructure has positioned Al-Qadsiah as potentially the 'fifth force' in Saudi football, capable of challenging the PIF-backed quartet of Al Hilal, Al Nassr, Al Ittihad, and Al Ahli.
